Retiring to Friuli: healthcare, climate, costs and residency

The table gives each area's median municipality; within every area, some places are better served than others.

• Trieste & Karst: hospitals close by, a mild January, the dearest square metre.
• Coast & Lagoon: the gentlest winter and the hottest summer.
• Hills & Plain: middling prices, frost in winter, heat in summer.
• Mountains: more space for the money, less December sun, a long way to hospital.

Grado's climate figures are shaky: its town hall is on the shore, outside the regional climate grid.

Drive times without traffic, from each town hall.

• 24-hour A&E: over thirty minutes' drive for 31 municipalities, mostly in the mountains.
• Heart attack and stroke: once paramedics recognise them, the ambulance heads straight for Udine, Trieste or Pordenone, each with a heart catheterisation laboratory and a stroke unit. One of the three is within half an hour for 87 municipalities.
• Closures: Cividale and Sacile lost their minor-emergency units (punto di primo intervento) at the end of 2025, and Gemona's no longer appears to be open.
• Across the border: from Tarvisio the nearest hospital is Villach, in Austria (26 min); from Sappada it is Pieve di Cadore, in the Veneto, whose A&E never closes (41 min).

A family doctor comes free with registration in the national health service. All municipalities: A&E and hospitals.

• Ice: counting the days with a sub-zero low, Trieste has 12 a year, Udine 67, Tarvisio 143. If you worry about slipping, that matters more than the average.
• Rising heat: at ARPA's Udine Sant'Osvaldo station, days above 30 °C went from an average of 40 (1991–2020) to 52 (2011–2025). On the plain, a cool house or air conditioning belongs in the budget.
• Sun: on 21 December the median mountain municipality gets 5 h 12 min of direct sunshine; 24 municipalities get less than five hours, all in the mountains.
• Snow on the ground: only a few places have a measured series; Forni di Sopra counts 84 days with snow cover (ARPA), Tarvisio 78 (Italian Air Force weather station).

• Prices: for €200,000, before purchase costs, the median municipality buys 120 m² in Trieste and the Karst, 245 m² in the hills or on the plain and 310 m² in the mountains (what €200,000 buys).
• Once you are resident and live there, the house pays no ILIA, the local property tax the Region introduced in place of IMU, unless registered as a luxury property (A/1, A/8 or A/9).
• If you stay resident abroad, it is a second home: any individual owner, resident in the region or not, may declare one such home online to the regional service and pay at most 0.70% rather than up to 1.06%. ILIA, municipality by municipality.

The 7% flat tax: in Friuli, the answer is no. Article 24-ter of Italy's income tax code (article 247 of the new code from 2027, with the same rules) grants a 7% substitute tax on all foreign income to people drawing a pension from abroad who move their tax residence to a municipality of up to 30,000 inhabitants (20,000 until 6 April 2026) in Sicily, Calabria, Sardinia, Campania, Basilicata, Abruzzo, Molise or Apulia, or in the areas struck by the L'Aquila (2009) and Central Italy (2016–2017) earthquakes. Friuli Venezia Giulia is not on the list: none of its municipalities qualifies, not even the smallest.

• Pharmacy in 206 municipalities of 215 (Ministry of Health register); the extreme case is Andreis, whose nearest pharmacy is 6.0 km from the town hall as the crow flies.
• Supermarket within a kilometre of the town hall, as the crow flies, in 115 municipalities, per OpenStreetMap: a careful map, not a census.
• Trains: a station with direct trains to Udine is within ten minutes' drive for 80 municipalities; the Udine–Villach Mi.Co.Tra. service stops within twenty minutes' drive for 45 municipalities.
• Flights: Trieste Airport is under an hour by car from 130 municipalities; Venice Marco Polo from 23 municipalities, around Pordenone and in the southern lowlands.

1. Codice fiscale (tax code): needed before any preliminary contract and for healthcare registration (how to get one).
2. Residency: an EU citizen staying more than 3 months without working registers with the town hall by showing sufficient means of their own and comprehensive health cover (step by step).
3. Healthcare for EU citizens: the S1 form, your home country's certificate of entitlement to treatment, means compulsory registration with the national health service, on the same terms as Italians, at the district health office. Without one: private insurance or voluntary registration.
4. British and American citizens need a visa first: for pensioners, the elective residence visa. For health cover, ask the local health authority before you move.
5. Tax: Italy has double taxation treaties with the UK and the US, among others; what they mean for your pension is for an accountant (commercialista) to say.
